Care Coordination

Supports sending notifications on the status of a patient’s care transitions to every member of their care team

Allows any participating organization to connect to the health information network a single time and exchange documents with any other participating organization

Addresses the need for a standard, shared way to manage consent forms and consent-related information

Helps healthcare providers share patient medication information at time of discharge with other care team members and organizations, including physicians, practices, pharmacies, hospitals, and others.

Enables providers in private-sector facilities and in VA facilities to request each other’s electronic health records for a veteran’s health information

Allows participating organizations to communicate with each other to find and send patient records quickly and electronically

Enables a participating organization to retrieve a patient’s authorization documents from the SSA and to then respond electronically to the SSA request by sending clinical Continuity of Care Documents

Supports the creation of an electronic referral system that enables electronic coordination between case management systems and healthcare providers’ electronic health records

Allows organizations to send data through MiHIN to the state, where the data is analyzed for social determinants of health information to promote overall public health.

Allows participating organizations to receive notifications of verified immunizations from an IIS in real time through the statewide health information network (HIN).
