The Tobacco Free Use Case establishes a bidirectional, electronic referral connection between healthcare providers and the State of Michigan’s tobacco cessation program, helping to connect patients interested in quitting smoking to free resources.
Healthcare providers can use their EHRs and MiHIN’s infrastructure to electronically send C-CDA Continuity of Care Documents (CCDs) containing embedded referrals to the case management eReferral system without interrupting normal workflow. This data exchange solution also supports case management coordinators who can electronically update a patient’s healthcare providers on the tobacco intervention using C-CDA Progress Notes.
This data exchange solution is relevant to any organization with providers that meet with patients and would like to start making tobacco cessation referrals or who are currently doing so through manual means.
