Medication Management
Interoperability
Medication reconciliation is the process of creating and maintaining an accurate, up-to-date list of all medications a patient is taking, including prescriptions, over-the-counter drugs, and supplements.
About this Use Case
Maintaining an accurate, up-to-date list of a patient’s medications, including prescriptions, over-the-counter drugs, supplements, and other remedies, is essential for optimal care, as medication regimens frequently change. Medication reconciliation involves listing, validating, and documenting this comprehensive medication information to ensure accuracy.
MiHIN supports these efforts by improving interoperability and advancing solutions that enable seamless sharing of medication data across care teams, directly supporting the HIT Commission’s Health IT Roadmap. By prioritizing medication reconciliation as part of its commitment to patient safety, MiHIN helps ensure that healthcare providers have the accurate information needed to deliver high-quality, coordinated care.
