One of the most challenging aspects of improving medication use safety and outcomes is to ensure that accurate information about a patient’s medication use is available across the health care continuum. In partnership with University of Michigan, we explored how health information exchange (HIE) can aid in maintaining an accurate and up-to-date list of each patient’s medications.
