Making Choices Michigan – ACP Community of Practice Overview

Introduction

The Advance Care Planning (ACP) Community of Practice is a network of individuals and organizations collaborating to provide and grow ACP services to all Michigan communities.  With foundational support provided by Making Choices Michigan (MCM), a strategic business unit of MiHIN, the network focuses on increasing statewide access to ACP and advance directives.  This group is committed to providing leadership, education, and guidance for those seeking to implement ACP in their organization and community.  The sharing of these statewide resources, connecting with others, acquiring resources needed to build an ACP framework, and fostering change in a sustainable fashion are at the core of this network.  Advocating for and empowering an individual’s right to align their own healthcare choices with their unique perspective of what matters most, and promoting strong patient advocate skills, can become the standard of care for all. To learn more and get involved, email acp@mihin.org
